Service
Description – NHS Halton
CCG is seeking to procure a safe and effective stabilisation, monitoring and
dosing anticoagulant therapy service in a community setting for non-complex and
stable complex patients. The service may also include warfarin initiation in
patients with stable AF for suitably qualified clinicians. The service will
minimise the potential adverse effects of warfarin by providing patients with
regular monitoring to stabilise their INR levels while continuing to maximize
the effective benefits of such treatment.

Supported Living Services
for Adults with a Learning Disability and/or Autism (Framework)
Description – Supported
living services refer to accommodation based services where there is a property
with multiple tenants (or a number of self-contained units in the same
vicinity) where the services being delivered to one individual are linked or
being provided in parallel to services being provided to another. Supported
living is not limited to services where the support is provided 24 hours-a-
day. The framework will be opened up and refreshed approx. every year to allow other
providers to join.
