Description - this contract is to provide housing related support to vulnerable young people
in Portsmouth at risk of Homelessness, with the overall aim of increasing
independence and the ability to move-on to lower supported or independent
accommodation, addressing the behaviours and issues which led to these risks in

West Leicestershire

Acute Delivery Services
(Pilot)
Description - The aim of the service is to reduce inappropriate admissions to hospital
by providing a rapid response home visiting service for patients who, if they
are not visited rapidly are at risk of admission. This scheme will cover
patients registered with all practices across West Leicestershire CCG (WLCCG). If
it is deemed at the visit that the patient requires urgent admission then the
GP organises this with the hospital and this results in a large number of
admissions in the afternoon, particularly for those patients needing hospital
transport.
