Description - This tender
comprises two lots and providers can bid for one or both;
1. Accommodation based and floating support service for teenage parents;
2. Supported lodgings for young people at risk, care leavers and looked
after children.
These services will provide support through sufficiently trained and
culturally aware support workers. The service will identify the individual
support needs of service users, address immediate needs and establish
links to other services that can provide longer term support.

Community Dermatology
Description - The CCG’s aim
is to commissioning localised ‘Integrated Community Dermatology’ services, that
support primary care and the specialist services. This service may also be
known as an ‘intermediate level service’ or a ‘Tier 2 Service’. Key components
of the service outlined in the specification include:
• Improves patient access to dermatological care, diagnostics and
treatment;
• Supports the patient to make informed decisions about care and
treatment options;
• Supports patient education and self-management of long term skin
conditions;
• Supports graduate and post graduate education of health care
professionals involved in the provision of dermatological services locally Key
objectives of this procurement:
• Coordination and leadership for the development of local dermatology
services in partnership with WHCCG
• Locality hubs for delivery of community based dermatological services
at an intermediate level which are outside the scope of services offered under
primary care contracts or arrangements
• Clinical triage and referral support in accessing specialist and
intermediate care services
• GP Advice and Guidance through e-referrals and tele-dermatology
