Continuing Healthcare AQP

Description – NHS Nene and NHS Corby Clinical Commissioning Groups require high quality domiciliary continuing healthcare services for appropriately assessed service users of Northamptonshire who live within or close to these geographical boundaries. We plan to implement these new contracts from September 2014.

Tier 3 Weight Management

Description – The aim of the Community Specialist Weight Management Service (CSWMS) is to improve the health and well-being of morbidly obese adults by supporting them to achieve and maintain a healthier weight and improve physical fitness, through making appropriate and sustainable lifestyle changes to their eating and physical activity habits.
The provision of a CSWMS is expected to:
— Support patients to make long-term lifestyle changes to manage their weight, improve their health status and their quality of life,
— Manage the access to bariatric surgery,
— Improve the screening process for those patients who are eligible for bariatric surgery.

City & Hackney CCG:
Super Patient Participation Group

Description – This service requires the provider organisation to deliver the ‘Super Patient Participation Groups’ (Super PPGs) from 11 August 2014 until 10 July 2016 for the six consortia (Well, Rainbow and Sunshine, Klear, North Hackney and South West) based in City and Hackney. Super PPGs consist of representatives of the practice based Patient Participation Groups. The groups meet on regular basis (current specification is 3 x year) to share information and discuss current issues affecting the health and wellbeing of the local population as well as local and national services.
